The Centers for Medicare and Medicaid Services have created a quality program which highlights specific areas of interest in your healthcare that potentially could optimize your health treatment outcome, improve your experience with physicians and create better access to care.  

Such areas of interest include:

  • Mammograms once yearly
  • Colorectal Cancer Screenings – some tests once completed do not have to be done again for 10 years!
  • Bone Density testing every 2 years

Medication Adherence

  • Are you taking your medications as prescribed?
  • Diabetic Care which includes Eye Exams yearly, foot exams yearly, and bloodwork.
  • Completing your Medicare Wellness exam yearly
  • Assisting with your transition to home from a hospital or rehab, this includes both emergency room and admitted stays.
  • Are you maintaining a healthy blood pressure?
  • Vaccinations History, are you up to date?
  • Ask your provider today if you are due for any updates

 

As a supplement to the Quality Program that the Centers for Medicare and Medicaid Services developed is a patient experience questionnaire that may be sent to you after your visit.

Such questions that could be asked include:

  • Did you experience any difficulty scheduling your appointment?
  • Did your doctor talk to you about falling, problems with balance, or walking?
  • Have you ever talked with your doctor about urine leakage?
  • Has your doctor spoken to you about your mental or emotional health?
  • Do you feel stressed?
  • Depressed?
  • Experience anxiety?
