In order to make your first visit run more smoothly, we ask to you take a few minutes to fill out the New Patient Packet that will be used to set up a patient chart.
Please bring your insurance card and a form of legal identification to your first visit.
As maintaining accurate patient records is a complex task that required cooperation between the staff and patient, we ask that you please bring your prescription bottles to appointments.
